Patient's Record CardThis card, used by the Caribbean Chronic Care Collaborative (CCCC), serves as a tool to monitor individual patients with chronic diseases. It shows any complications related to hypertension, heart disease, stroke, retinopathy, neuropathy, diabetic foot, amputation, high cholesterol, and tobacco and alcohol use. It also records information on visits, measures, exams, tests, treatment, and vaccines and current indications on blood pressure, height/weight, body mass index (BMI), foot and eye, blood glucose/ A1c, lipid profile, electrocardiogram (EKG), medication and dose.
