Questionnaire to Assess the Diagnosis and Treatment of Chronic Diseases

This questionnaire is designed to obtain data for assessing diagnostic and treatment services for chronic noncommunicable diseases (CNCDs) and conditions constituting risk factors for these diseases: cancer, cardiovascular diseases, chronic obstructive respiratory disease, diabetes, hypertension, nutrition, physical activity, smoking cessation, stroke, weight control, and any other condition/disease of importance to the respective country.
