TÉRMINOS DE REFERENCIA

1. To develop & serve as a demonstration site for the education of persons with diabetes, as a key strategy in addressing quality of care to achieve improved clinical & social outcomes. 
2. To develop and test the applicability and efficacy of a theoretical and practical model program of diabetes education, accessible to health professionals and other healthcare team members working at primary care level in the Region, in the prevention, control and treatment of diabetes. 
3. To undertake training courses, accessible to health professionals & other members of the health teams as well as health care providers throughout the Region, in the prevention, control & treatment of diabetes. 
4. To prepare & facilitate the distribution of the new educational material devoted to update diabetes knowledge and use of diagnostic procedures & therapeutic tools applicable to care and treatment of people with diabetes.
